Extended Fill aftercare: first 48 hours & care tips
By Elva Medical Aesthetics | August 11, 2026
Right after your Extended Fill, the one rule that protects your retention the most is simple: keep your lashes dry and oil-free for the first 24 to 48 hours. We do Extended Fills in about 20 minutes, and they’re meant for days when you’ve got less than 50% of your lashes left, so that “fresh full” look comes back fast. Now your job is to not undo it with steam, sweat, rubbing, or oily skincare. This guide is what we tell our Hamilton clients, especially on Hamilton Mountain, when they want their fill to last without extra morning steps.
Quick context: An Extended Fill is for catching up when you’re more grown out than a regular fill. We add more coverage so things look even again, then aftercare keeps those bonds strong so you don’t see early gaps.
Your first 24 to 48 hours after an Extended Fill: do this, skip that
Think of the first two days as “hands off, face calm.” The adhesive needs time to fully cure, and Hamilton weather can make that harder. Humid Hamilton summers, sweaty commutes, and pool days can sneak up on you fast.
Do
- Keep lashes dry. No face-in-the-shower water, no soaking, no swimming, no hot tubs.
- Sleep on your back if you can. If you’re a side sleeper, try to keep your face off the pillow.
- Use a clean spoolie to gently brush through, especially if you notice a lash flipping after sleep.
Skip
- Steam and heat: hot showers pointed at your face, saunas, steam rooms, and leaning over a pot of boiling water.
- Heavy sweat workouts. A light walk is usually fine, but save intense training for after that 24 to 48 hour window.
- Oil-based skincare around the eye area and oil-based makeup removers. Oils break down bonds and shorten wear.
- Rubbing, picking, or “testing” the lashes. If something feels off, we’d rather you tell us than tug at it.
Hamilton Mountain reality check: If you’re hopping on the HSR or driving the Red Hill Valley Parkway after work and you’re tempted to squeeze in a sweaty workout right after, push it to the next day. Retention is usually better when we protect that first 24 to 48 hours.
Days 3 to 7: keep them clean, and stop the little habits that cause gaps
Once you’re past that first 48 hours, cleanliness is what keeps your lash line looking even. A lot of people think washing will make extensions fall out, but it’s usually the opposite. Makeup residue, oils, and skin buildup can make lashes twist, stick together, and shed unevenly.
If you wear makeup
Keep eyeliner and shadow away from the lash base when possible. Skip waterproof formulas near the eyes, since they usually need oily removers and extra rubbing. And please skip mascara on extensions. It tends to clump at the base, makes cleaning harder, and you’ll be tempted to scrub it off later.
If you’re in the gym or outdoors
Hamilton summers get humid, and sweat plus sunscreen migration is a common combo. Cleanse your lashes after a workout or a long outdoor day. In winter, the air is dry and people crank the heat, which can leave lashes looking a bit “crisp” if you’re not washing away buildup. Gentle cleansing helps year-round.
- Don’t pick at a lash that feels “loose.” That’s how small gaps turn into obvious gaps.
- Don’t use an eyelash curler. It can snap natural lashes and crush extensions.
- Do brush after cleansing and after sleep, just enough to separate and reset.
The lash bath routine that actually keeps an Extended Fill looking full
If you take one habit from this post, make it a quick lash bath. We usually recommend a foam lash cleanser or a water-based, oil-free cleanser. Nothing harsh. No scrubbing. The goal is clean skin at the lash line, not squeaky-dry lashes.
Daily (or most days) in 60 seconds
- Wet your face and cleanse around the eyes gently (after the first 24 to 48 hours).
- Apply a small amount of foam cleanser to the lash line and lightly work it through with clean fingertips.
- Rinse well. Pat dry, don’t rub. A gentle fan or cool blow-dry at a distance is fine.
- Brush with a clean spoolie to separate.
Products that usually cause trouble
- Oil cleansers, cleansing balms, and heavy eye creams that migrate.
- Cotton pads and Q-tips at the lash base (they snag and leave lint).
- Waterproof makeup near the eyes.
And yes, it’s normal to shed a few natural lashes each day. We’re working with your natural growth cycle. Aftercare just makes that shedding look softer and more even.
When it’s time to message us, plus the 2 to 3 week touch-up rhythm
If something feels pokey, twisted, or just not right, let us know early. A quick check beats living with irritation or trying to “fix” it yourself. A lot of first-time lash clients tell us the education is what settles their nerves, and our team keeps that same energy after your appointment too.
"The first time I got my lashes done she explained everything to me which helped ease my anxiety of trying something new."

For maintenance, most people book a touch-up every 2 to 3 weeks. That’s the sweet spot for keeping fullness without chasing gaps. If you’re past that and you’ve lost a lot of extensions, that’s when an Extended Fill makes sense again, because it gives us time for fuller coverage when you’re under that 50% mark.
Local note: If you’re coming in from Stoney Creek, Dundas, Ancaster, Burlington, Flamborough, or Waterdown, we’ll help you keep your schedule simple with quick touch-ups near the Hamilton Mountain (Concession Street area) routine you can actually stick to.
Ready to keep your set looking even? Start with Extended Fill (20 minutes, starting at $20), then we’ll keep you on a rhythm that makes sense for your lashes.

A quick reset if you’ve had retention issues before
If you’ve tried extensions elsewhere and they didn’t last, don’t assume lashes “just don’t work” for you. We hear this a lot, and it’s usually one of a few fixable things: oils at the lash line, makeup removal habits, steam and sweat too soon, or stretching appointments too far apart. The good news is that once you get consistent with cleansing and you keep your fills on that 2 to 3 week schedule, lashes tend to wear better and look more even between visits.
